Palatine tonsils are the most frequent location of primary mucosal melanoma; however, tonsillar metastasis of cutaneous malignant melanoma are infrequent. The tipical clinical presentation is odynophagia and dysphagia, caused by the growth of a tonsillar mass. Clinical Case: A year-old patient with a clinical history of a cutaneous melanoma in her upper limb left scapula T3bN0M0-stage II , treated with surgery and inmunoterapy, with brain, liver and bile ducts metastasis stage IV diagnosed and treated for five months with immunotherapy with IFN alphab.

She went to the emergency room for recurrent unilateral tonsillitis associated with bleeding, without improvement despite broad-spectrum antibiotic treatment and anti-inflammatories. Given the clinical persistence, it was decided to perform a biopsy of the lesion with the result of a high suspicion of metastatic melanoma. Given the recurrence of episodes of oral bleeding, it was decided to perform a tonsillectomy.

The anatomopathological result confirmed the histology compatible with metastasis of a cutaneous melanoma. The patient presented an adequate postoperative recovery and continues her immunotherapeutic treatment without new lesions at the oropharynx, remaining stable in her disease to date.

Discusion: Distant metastasis of a cutaneous melanoma in the oropharynx are very rare. The surgical treatment provides the double benefit of histological confirmation and the containment of symptoms, so it should be considered without delay. Cutaneous Melanoma with Tonsillar Metastasis.
